To allow us to prepare your Self-Assessment tax return accurately and on time, we need to gather some information from you. This form is straightforward — you will only be asked about the income sources and circumstances that apply to you.

Please return this form no later than 31st August 2026. The sooner we receive it, the sooner we can get things moving.
If you are self-employed, please note this form covers your personal tax position only — your business records will be dealt with separately.
Do not worry if you are unsure about some of the figures — provide what you can and we will do the rest.

Making Tax Digital (MTD) for Income Tax — What You Need to Know

HMRC is rolling out MTD for Income Tax in stages. If you are a sole trader or landlord, the following thresholds apply based on your gross qualifying income:

From April 2026 — mandatory if income exceeded £50,000 in 2024–25
From April 2027 — mandatory if income exceeded £30,000 in 2025–26
From April 2028 — mandatory if income exceeded £20,000 in 2026–27
We will review your position individually and let you know whether and when MTD applies to you. No action is needed from you at this stage.
